« Le programme Best Care met l'accent sur la gestion intégrée des maladies chroniques et promeut des normes fondées sur des données probantes à tous les niveaux de soins, et a obtenu des résultats positifs pour d'autres maladies chroniques telles que l'asthme et la maladie pulmonaire obstructive chronique. Le programme Best Care a amorcé des travaux sur l'insuffisance cardiaque en 2016 dans le cadre d'un projet pilote, avec un lancement complet en 2018. La région a connu un succès précoce dans l'amélioration des résultats, notamment la qualité de vie des patients et la réduction des hospitalisations et des visites à la salle d'urgence. »
