About Best Care

Our Vision 

Our vision is to improve the lives of those living with chronic diseases and reduce the health system costs, working in close partnership with Canadian patients, primary care providers and health professionals. 

What is Best Care?

Across Canada, Best Care works directly with primary care providers to improve life for people living with chronic diseases. We work wherever patients normally receive their primary care, so it’s convenient for them.  
 
The Best Care model was developed by frontline health providers who understand the challenges of chronic disease management, impact on patient lives and the burden on the health system. It works by integrating a Certified Integrated Chronic Disease Clinician, who is also a case manager, into the patient’s health care team, working closely with their primary care provider.  
 
As a self-funded model, Best Care has a proven impact, delivering more than $100 million in savings to the health system and transforming the care experience for thousands of patients.

Primary care, connected 

One standard program means all patients receive the best care. Our model is scalable and efficient, plus builds capacity and encourages innovation. 

Advancing care at every step

With a team-based care approach focused on the patient, Best Care’s model aims to positively transform physical, mental and emotional health. 

Accountability through data 

Quality of care is measured every visit, through performance measurement and benchmarking. We maintain one common electronic health record across all providers and use digital tools to provide a better patient experience. 

A preventative and proactive approach 

Our approach empowers patients, primary care providers, specialists 

and educators we work with to better manage chronic diseases. We also support collaborative interprofessional leadership among the professionals we work with, building capacity in the system. 

A focus on quality of life 

By focusing on integrating with primary care providers, we can prevent hospitalizations, and unscheduled and emergency visits. That not only reduces the burden on the health system but also improves life for people living with chronic diseases and their loved ones.

About Asthma Research Group Inc. (ARGI) 

Asthma Research Group Inc. (ARGI) is a not-for-profit organization, originally established to support individuals living with asthma through evidence-informed care. Building on this foundation, ARGI has expanded through the Best Care program to deliver integrated chronic disease management for people living with multiple long-term conditions.
