Team-based care, close to home
Best Care is transforming chronic disease management in Canada by delivering high-quality care close to home, improving lives for patients, providers and our health system.
How does it work?
Best Care helps people living with chronic health conditions better manage their health. Harmonizing care for better chronic disease outcomes
Our approach is holistic and person centred. That means we consider how various chronic health conditions – from lung diseases to diabetes, kidney disease, heart failure and more – may be interrelated. That supports better care and better outcomes.
Delivering on all quality standards and best practices
Standardized care aligns with best practices defined by provincial, national, and international guideline standards. Patients are empowered to play an active role in their own care journey through educational support, personalized care plans and self-management training.

Office of the Auditor General of Ontario
"The Best Care program focuses on integrated chronic disease management and promotes evidence-based standards through all levels of care, and has seen positive outcomes for other chronic diseases such as asthma and chronic obstructive pulmonary disease. The Best Care program began work on heart failure in 2016 as a pilot, with full launch in 2018. The region has experienced early success in improving outcomes, such as patient quality of life and reduced hospitalizations and emergency department visits."

The Conversation
"When evaluating programs in terms of health economics, a program is dominant when it improves patient outcomes and costs less than the alternative standard of care: in other words, better care at a lower cost."

Pulmonology Advisor
"associated with a change in disease trajectory from increasing acute health service utilisation, to an immediate and sustained long-term reduction in hospitalisation and ED visits."

COPD IDM Christine Jenkins Editorial
